New Patients

Thank you for choosing Comprehensive Orthopedics Sports Medicine for your orthopedic care. We would like your first visit with us to be a positive experience. The information below will help our staff process your personal information, and enable your doctor to properly evaluate your condition.

Please bring the following items with you to your appointment:

• Insurance card
• Picture ID: drivers license
• Referral from your primary care physician, if applicable
• Payment for today’s services
• A complete list of the names and dosage of all the medications that you take
• All x-rays, MRIs or other imaging films (including written report)
• Information about what treatments and medications have been tried in the past
• Medical records from other physicians that you think may be helpful
• List of questions you want to ask your doctor

Complete a Patient Registration Form before coming to your first appointment if at all possible.

For your convenience, you can fill out a Patient Registration Form and bring it to your appointment.

Please dress comfortably.

The doctor will need to examine the area that is bothering you. You may also need to get an x-ray. You may not need to wear an examination gown if you wear or bring clothing that provides access to the affected area, such as shorts, short-sleeved shirts, etc.

It is our policy to collect all co-pays and fees not covered by your insurance at the time of the visit.

If you do not have insurance, full payment is expected at the time of the visit. We accept personal check, MasterCard, Visa, American Express and Discover.

If you are a self-pay patient it is our policy to collect a payment at the time of your visit.

Discount arrangements can be established with the practice manager for patients who pay for services at the time of visit.
