Billing & Insurance

 

 

We know that medical bills can be confusing. We hope the following information will help you better understand your bill and ensure a smooth billing process.

 

 

Billing Your Insurance

After your visit with us, we will submit a bill (also called a claim) to your insurance company if you have insurance. This bill will list the services we provided to you. Your insurance will use the information in the bill to pay us for those services.

 

When the insurance company pays us, they may send you a report called an Explanation of Benefits, or EOB. The EOB shows what your insurance company paid for and what it did not pay for, and why. An EOB is not a bill.

 

 

Your Statement from Us

If you have insurance, you may receive a statement that shows how much we billed your insurance company. You do not need to pay the amounts listed at that time.

 

After your insurance company pays us, you need to remit any balance due.

 

 

Your Bill from Us

You may receive a bill or several bills after your visit with us from the following departments or individuals:

  • One from your doctor that includes exam charges
  • One from the clinic or hospital that includes fees associated with procedures, non-physician personnel, exam room and equipment charges and medical supplies
  • One from the Radiology Department for x-rays

Your bill will show the balance due at this time.

 

This sample bill is a visual guide that will help you understand information on your bill. If you have any questions about your bill, please feel free to call the clinic’s billing office at 801-984-3418, Monday through Friday 8 a.m. to 5 p.m. You are always welcome to call the clinic with any questions.
